Cumbria Partnership NHS Foundation Trust provides a wide range of services within the community across Cumbria, in people's homes, community centres, clinics and hospitals. The Community Teams in each Locality are working to help people remain in their own homes and enhance independence.

Here are the community services we provide:

Diabetes - provide information and support as well as medical care for people diagnosed with diabetes

Community Nursing - from dressing clinics and drop-in health clinics to medication support, crisis intervention and case management. These services are provided locally by your community nursing team for housebound patients

Specialist Nursing Services - working in consultation with your Chronic Obstructive Pulmonary disease service and heart failure clinic, this service provides you with assessment and treatment as well as information and support

Liaison Nursing - the specially trained team will help you move from Acute Care in a hospital setting to Community care in a seamless way, providing support with planning your discharge from hospital. This service also provides continuing care assessments for you while you are in hospital

Patient Care Facilitators - these specialists can assess your health and social care needs, and co-ordinate your care

Early Supported Stroke Discharge Service - the support is available to help people return home as soon as possible after recovering from a stroke in hospital

Podiatry - as well as providing general foot care, the service also works with other services including vascular, diabetes and biomechanics

Short Term Intervention Service (STinT) - provide a rapid response to avoid you having to go into hospital or a care home when it's not necessary. They will manage your case and work with you on your rehabilitation

Chronic Fatigue Syndrome/Myalgic Encephalitis Service - provide treatments and assessment with links to consultants in Newcastle. There is a monthly consultant clinic at Keswick Community Hospital

Stop Smoking Service - is available to anyone over 16 providing clinic, group or telephone support

Speech and Language Therapy Service - is for people over 18 available in out patient or in your own home

Physiotherapy Service - provides clinics in the Physiotherapy department at community settings including community hospitals and clinics

Occupational Therapy - is available for over 18. Though based at Penrith and Alston Hospitals, they provide their services through Day Service and community settings

Dietetics - dietary and nutritional assessment and treatment for you in your own home, as an out-patient or in-patient for over 18 year olds

Continence - specialist continence team provide their services on a community basis

Sexual Health Services - provide medical terminations, assessment, treatment and screening of sexual infections, contraceptive advice and is available to everyone aged over 12 years old

Respiratory Service - provide specialist reparatory care and assessment in patient's homes and clinics. The also provide pulmonary rehabilitation for the adult population.

Tissue Viability Services - specialises in the prevention and management of wounds and work with people in their own homes

Cardiac Rehabilitation Services - work with adults following a heart attach in their own homes, group and clinic settings

Community Gym - provides physical exercise programmes specific to each persons treatment and needs.
